A set up fee per custom blank is 5$ + printing cost for the specific size:
$1/pair for sizes 4mm-12mm
$1.50/pair for sizes 14mm-18mm
$2/pair for sizes 20mm-24mm
$4/pair for sizes 26mm-30mm
